Referral Services-

Referrals can be to Horizon’s by self, parent, court, SRS, Foster care, or Physician. To make an intake appointment, call in. During the appointment therapist may suggest programs. Individual must be determined eligible before accessing any programs. School based services have proven to be very beneficial. Schools cannot refer to Horizon’s but can provide parents/guardians with information. Schools are encouraged to have the parents/guardians to call in and set up an informational meeting with Horizons staff at the school.
