With a meeting of administrator, special education teacher, general education teacher, and parent

 

 

 

Written and signed amendment form

 

 

 

Prior written notice of any changes (blue form)

 

 

 

Written consent if a substantial change in placement or material change in service (blue form)

 

 

 

The IEP team must be informed

 

 

Not needed

For specific day-to-day adjustments in instructional methods and approaches made by the general or special education teacher to assist the student to reach goals.
